Autism Advisory and Intervention Service, Primary Behaviour Support, SENIS DS, SENIS MLD, SENEYIS, and Sensory Service

AIMS:

Identify key partners in transition.
Outline the core aspects of support for effective transition.
Use child centred planning to develop bespoke transition plans.
To reflect on issues and challenges around the transition of children with SEN and how it affects children, their families and educational professionals.
